Vaccines for ages 5-11 

Children 5-11 years old are now eligible to receive the Pfizer-BioNTech (Pfizer) COVID-19 vaccine. 

Green County Public Health is working in partnership with Hometown Pharmacy and local school districts to offer on-site Pediatric Pfizer COVID-19 vaccine clinics for students during the week of November 15th. Parents and guardians will receive more information about these clinics from their child’s school. 

SSM Health is now scheduling appointments for COVID-19 vaccinations for kids ages 5-11. Vaccines are available through the following ways: 

• During previously scheduled Pediatric Department appointments in Monroe 

• COVID-19 vaccine only appointments 

• SSM Health is offering a mass pediatric vaccination clinic for ages 5-18 on Saturday, November 13, from 8am – 1pm in Monroe. Appointments are required. 

 

Appointments can now be scheduled through MyChart. For families without access to MyChart, vaccination appointments are available by calling 608-324-1815. This line is available Monday through Friday from 8am to 4:30pm. 

 

Green County Public Health Vaccine Clinics 

Walk-in clinics are available at Green County Public Health on Mondays, Tuesdays and Thursdays from 8am – 4pm. We are offering all three COVID-19 vaccines to those ages 12+. During these walk-in clinics community members can receive their first dose, second dose, their third dose, or booster dose of the COVID-19 vaccine. Those coming in for their second dose, third dose or booster dose should bring their CDC COVID-19 vaccination card. For a list of who is eligible to receive a booster dose or third dose of the COVID-19 vaccine please visit the Wisconsin Department of Health Services website. 

SSM Health Monroe Clinic Medical Group Vaccine Clinics 

In Monroe: SSM Health Monroe Clinic Medical Group is offering COVID-19 vaccines to individuals ages 12+ on Mondays through Fridays from 8am to 4pm. To schedule, please call 608-324-1815. This line is available Monday through Friday from 8am – 4:30pm. 

 

At our branches: SSM Health Monroe Clinic Medical Group is offering COVID-19 vaccines at our branch locations in Albany, Blanchardville, Brodhead, New Glarus and Freeport. Individuals must be 18+ and available to return in 28 days for the second dose. Appointments are required and can be made by calling the specific branch:

  • New Glarus: 608-527-5296 
  • Freeport: 815-235-1406 

A parent or guardian of those under 18 should call to schedule the appointment or accompany them at their vaccination appointments. Individuals do not need to be a current patient and insurance is not required to receive the vaccine at SSM Health. 

Boosters: SSM Health Monroe Clinic Medical Group is offering booster doses for the following qualified patients as defined by the CDC: 

  • People 65 years of age and older 
  • Residents of long-term care and congregate living facilities 
  • 18-64-year-olds with certain underlying medical conditions 
  • Individuals whose occupations put them at high risk for exposure to COVID-19

As we also enter into flu season, please continue to be vigilant in protecting yourself and others from spreading illness. This can be done by wearing a mask, maintaining social distance, practicing frequent hand washing, and staying home when you are sick.
